🫁 Why drowning CPR puts breaths first
A drowning victim goes into cardiac arrest because their body ran out of oxygen, not because their heart suddenly failed — so getting air back into the lungs is the single most important thing you can do. That is the opposite priority from the "hands-only" CPR taught for a collapse on dry land, where the blood is still oxygen-rich and compressions alone can buy time.
In November 2024, the American Heart Association and the American Academy of Pediatrics issued a focused update making this explicit: anyone in cardiac arrest after drowning should receive CPR with rescue breaths, and trained rescuers should start ventilation as early as safely possible. For a struggling but breathing child, rescue breaths may even begin the moment they are reached. The takeaway for families is simple: in the water, air comes first.
This is why a drowning rescue does not end when the child reaches the edge. A submerged swimmer can look fine one minute and stop breathing the next. Knowing the steps ahead of time is part of the last layer in our five layers of drowning protection — emergency readiness.
🧭 The CPR steps after a water rescue
Once the person is out of the water, the sequence is check, call, breathe, then pump — and keep going without long breaks. Run it top to bottom, fast, and let a 911 dispatcher coach you the whole time.
1. Check. Tap and shout — "Are you okay?" Look for normal breathing for no more than 10 seconds. Gasping is not normal breathing.
2. Call. Point to someone specific: "You — call 911 and bring the AED." If you are alone with a child and no phone, give about 2 minutes of CPR first, then call. Put the phone on speaker so your hands stay free.
3. Breathe. Tilt the head back, lift the chin, and give 2 rescue breaths, each about one second, watching for the chest to rise. For an infant, cover the nose and mouth and give small puffs.
4. Pump. Push hard and fast in the center of the chest, 100–120 compressions per minute, letting the chest fully recoil each time. After every 30 compressions, give 2 breaths.
5. Continue. Keep 30:2 cycles going until the person breathes normally, an AED is ready to use, or trained responders take over. Follow the AED prompts as soon as one arrives, drying the chest first.

👶 Compression depth and breaths by age
The rhythm stays the same across ages — 30 compressions to 2 breaths for a lone rescuer — but the hands, depth, and breath size change with the size of the child. Getting these right matters, which is exactly why a hands-on class is irreplaceable.
Adult or older child: heel of two hands, center of chest, about 2 inches deep. Child (roughly 1 year to puberty): one or two hands, about 2 inches or one third the chest depth. Infant (under 1 year): two fingers or two thumbs, about 1.5 inches deep, with gentle puff-sized breaths. In all cases, push fast, let the chest come all the way back up, and minimize pauses.
A drowning victim often has water and vomit in the airway. Turn the head or roll the body to clear the mouth if needed, then continue. Do not waste time trying to "push water out" of the lungs — that delays the breaths and compressions that actually help.
🩺 After the rescue: get checked, even if they seem fine
Any child who was submerged, needed rescue breaths, or swallowed a lot of water should be evaluated by a medical professional, even if they recover and look normal. Rarely, breathing problems can appear hours after a water incident. The popular terms "dry drowning" and "secondary drowning" are not medical diagnoses, but delayed respiratory distress after submersion is real and worth watching for.

Knowing CPR is essential, but it is the layer you hope never to use. The layers that keep the emergency from happening in the first place are barriers, supervision, life jackets, and swim skills.
